Effective Date: October 21, 2026

Your Information. Your Rights. Our Responsibilities.

THIS NOTICE DESCRIBES HOW MEDICAL AND BEHAVIORAL HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, HOW YOU CAN GET ACCESS TO THIS INFORMATION, AND SPECIAL CONFIDENTIALITY PROTECTIONS THAT MAY APPLY TO SUBSTANCE USE DISORDER RECORDS. PLEASE REVIEW IT CAREFULLY.

Manzi Behavior Health is committed to protecting the privacy and confidentiality of your health information. We are required by applicable law to maintain the privacy and security of protected health information (“PHI”), provide you with this Notice of Privacy Practices, and follow the privacy practices described in the notice currently in effect.

Certain substance use disorder (“SUD”) records may receive additional protection under federal law, including 42 CFR Part 2.

Your Rights

You have certain rights regarding your health information. Subject to applicable law, these rights may include the right to:

Obtain a copy of your medical record.
You may request to inspect or receive an electronic or paper copy of your medical record and other health information we maintain about you. We will provide access as required by applicable law.

Ask us to correct your health information.
If you believe information in your medical record is incorrect or incomplete, you may ask us to correct it. We may deny certain requests when permitted by law and will explain the reason when required.

Request confidential communications.
You may ask us to contact you in a particular way or at a particular location, such as using a specific telephone number or mailing address. We will accommodate reasonable requests as required by law.

Ask us to limit what we use or disclose.
You may request restrictions on certain uses or disclosures of your health information. We are not required to agree to every request, except in circumstances where the law requires us to do so.

If you pay for a healthcare service or item completely out of pocket, you may ask us not to disclose information about that service or item to your health plan for payment or healthcare operations when applicable law requires us to honor that request.

Receive an accounting of certain disclosures.
You may request a list of certain disclosures of your health information made during the period permitted by law. Certain disclosures, including some made for treatment, payment, and healthcare operations, may not appear on the accounting.

Receive a copy of this Notice.
You may request a paper or electronic copy of this Notice at any time, even if you previously agreed to receive it electronically.

Choose someone to act for you.
If you have given someone medical power of attorney or another person has legal authority to act on your behalf, that person may exercise your privacy rights as permitted by law.

File a privacy complaint.
You may file a complaint if you believe your privacy or confidentiality rights have been violated. You will not be retaliated against for filing a complaint.

Your Choices

For certain health information, you may tell us your preferences regarding what we disclose.

When permitted by law, you may tell us whether we may share certain information with family members, close friends, caregivers, or others involved in your care or payment for your care.

Certain uses and disclosures require your written authorization. We will obtain your authorization when required by law, including for certain marketing activities, the sale of PHI, and other uses or disclosures not otherwise permitted by law.

You may revoke an authorization in writing, subject to applicable law and to the extent that we have not already acted in reliance on the authorization.

How We May Use and Disclose Your Health Information

Subject to applicable federal and Arizona law, we may use or disclose health information for purposes including:

Treatment.
We may use and disclose health information to provide, coordinate, or manage your healthcare and treatment.

Payment.
We may use and disclose health information as permitted to bill and obtain payment from health plans or other responsible parties for services provided to you.

Healthcare Operations.
We may use and disclose health information for lawful healthcare operations, such as quality improvement, compliance activities, credentialing, auditing, and certain administrative functions.

We may also use or disclose health information when permitted or required by law for purposes such as public health and safety activities, health oversight activities, responding to certain legal proceedings, reporting suspected abuse or neglect when legally required, preventing or reducing a serious threat to health or safety, workers' compensation, and other purposes authorized by law.

We will apply any additional protections required by Arizona law when those protections are more restrictive than federal requirements.

Mental and Behavioral Health Information

Mental and behavioral health information is sensitive and may receive additional protections under federal and Arizona law.

Manzi Behavior Health will maintain the confidentiality of medical and behavioral health records and will disclose such information only as permitted or required by applicable law or pursuant to a valid authorization when authorization is required.

Substance Use Disorder Records and 42 CFR Part 2

Certain records relating to the identity, diagnosis, prognosis, or treatment of a patient receiving substance use disorder services may be protected by 42 CFR Part 2 in addition to HIPAA and applicable Arizona law.

When Part 2 applies, Manzi Behavior Health will obtain patient consent for uses and disclosures when required by law and will use or disclose Part 2 records without consent only when permitted by applicable law.

A patient may provide consent allowing certain uses and disclosures of Part 2 records for treatment, payment, and healthcare operations as permitted under federal law.

Part 2 records, or testimony describing information contained in those records, generally may not be used or disclosed in civil, criminal, administrative, or legislative proceedings against a patient unless the patient provides specific written consent or a court order meeting applicable federal requirements authorizes the use or disclosure.

Manzi Behavior Health will not use Part 2 records to investigate or prosecute a patient except as specifically permitted by federal law.

You may revoke a consent involving Part 2 records as permitted by applicable law.

Our Responsibilities

Manzi Behavior Health is required by law to maintain the privacy and security of your protected health information.

We will notify you as required by law if a breach occurs that may have compromised the privacy or security of your information.

We must follow the duties and privacy practices described in the Notice currently in effect.

We will not use or disclose your information other than as described in this Notice or otherwise permitted or required by law unless you provide written authorization when authorization is required.

Additional protections may apply to certain categories of information, including substance use disorder treatment records and other specially protected information.
